Dr. Mighell’s patient is a 70-year-old woman with a history of myasthenia gravis who presented to his practice with severe left shoulder pain that had not responded to conservative treatment. On examination, she was found to have pseudoparalysis and weak external rotation. Radiographs showed an absence of cartilage on the glenohumal joint, while an MRI showed cystic formation on the humeral head and a compromised rotator cuff.
